There are different ways to join the Registry:

Register on line.

The quickest and most efficient way is to register online. This is a two-step process:

• First, you submit your contact information.
• Then, you complete a second form, providing information about the individual at risk for anaphylaxis. If you're not a patient or family member (e.g. parent), you'll be asked what topics you're interested in and what industry you belong to.

Confirmation of your submission and your PIN number and password will be sent by email so that you can update your profile on line.

Download the Registry form (PDF format).

Complete the form and return it by fax or mail. Anaphylaxis Canada contact information is on the form.
